Forms Required for Student Athletes
The following forms are required for each student athlete participating in sports at Toledo Christian in grades 7-12. Student athletes under grade six will be asked to turn in the appropriate form(s) by their individual coach, based on the league/sport the team is participating in.
OHSAA Pre-participation Physical Evaluation Form
The Athletic Physical Form is required by the start of the first sport that the student athlete will be participating in for the school year. All forms should be turned into the Athletic Office.
Ohio Department of Health Concussion Information Sheet
This form can be found at http://www.ohsaa.org/medicine/Concussions/ODH-ConcussionInfoSheet.pdf and is required for every student athlete for each sport that they play in the State of Ohio. This form must be signed by the student athlete as well as the parent and turned in by the beginning of the sports season.
Emergency Medical Form
This form may be found at http://toledochristian.com/wp-content/uploads/2014/01/Home-Page_Parents_Forms_Emergency-Medical-Form.pdf and is required by each student at TCS. The Athletic Office will obtain a copy of the Emergency Medical Form from the Nurse’s Office for its athletes. If your student does not have this form on file, they will not be allowed to participate in their sport.
Student Driver Form
Student Volunteer Driver Form – This form is needed if you would like to approve your student to drive to games within the Toledo Area ONLY. This would include games such as: Ottawa Hills, Cardinal Stritch, or Northwood.
Parent Driver Form
Parent Volunteer Driver Form – This form is needed if you plan on being a volunteer driver for our sports teams. Remember that this will count toward your volunteer hours.
